Prior to each scheduled meeting, you must complete a written self-assessment form.  The forms for each meeting can be found in the “Individual Meetings” folder on Box (“Files for Students” > “Individual Meetings”).  Please print and complete the form and bring it with you to your meeting. You may write in or type your responses on the form.  If you choose to write your responses, please be sure to write legibly in black ink.

 

Please let us know if you cannot access the forms in Box for any reason.

 

End of Semester and Exit Interview meetings will be conducted individually, with both the Program Director and Assistant Program Director, as scheduling allows. At the conclusion of the meeting, the program will provide a written response and all parties will attest through signature to reviewing the form. A signature does not indicate agreement with the information included. A signed copy of the form will be scanned and uploaded to your Online Portfolio.
