An online portfolio has been set up for each student. This portfolio is designed to capture the critical elements of your academic work. 

Access: You will be sent a link to your portfolio in Box. You can log into Box from any web browser by accessing http://box.uncg.edu and your UNCG login and password. Google Chrome is the preferred browser for Box, but you should be able to access from any browser.

 

Below is a list of files that should be kept or maintained within your portfolio. Items to be uploaded are listed below with the responsible party indicated. Following graduation, all files contained in your folder will be kept in your permanent program file for accreditation audits and/or verification of training.

 

To be uploaded by Students:

Capstone Files

  • CITI training or Human Subjects Research Workshop Certification
  • Summer Assistantship Proposal (signed by student + PD)
  • Project Proposal (signed PDF by student + committee)
  • IRB files, as appropriate: Submission (PDF), Consent Forms (PDF), Approval (PDF)
  • Capstone Final Report
  • Capstone Slide Presentation

Non-Clinical Activities

  • An Excel file of updated non-clinical activities is to be updated at the end of each semester

  • Logbook Files and Case Summary Sheets by rotation site
    • Logbook database for each clinical rotation
    • Scanned case summary sheets

Course Documentation

  • A summary of your grades for each enrolled GEN course will be uploaded at the end of each semester 

Rotation Requirements

  • Background check, drug screen and other site specific requirements per Assistant Program Director

Rotations

  • Evaluation
    • Final rotation evaluation from each site
  • Rotation meetings with Assistant Program Director
    • A copy of your rotation meeting summaries will be uploaded

Self-Assessments & Exit Interview

  • You will complete a self-assessment form at the end of each semester and meet with the Program Director and Assistant Director. A copy of the signed, completed assessment will be uploaded after your meeting.
  • Prior to graduation, you will complete the Exit Interview form and meet with the Program Director and Assistant Director.
